Medical Application Form

Please fill a separate application form for double occupancy.

Accommodation is provided only for 2 persons in each suite.

All the guests are requested to forward their latest medical reports.

Guests are requested to submit their ID proof at the time of admission.

Guests above 60 years, guests with severe respiratory problems, hypertension, cardiac history should enclose their recent cardiac status evaluation reports.

Pregnant ladies, young children below 12 years will not be admitted in the resort.

Admission will not be granted to guests with severe skin problems, infectious diseases and also to physically very weak people.

Guests are suggested not to carry/ wear / bring along any valuable jewellery items for the course of their healing stay at Pema.Guests are responsible for the safety of the valuables incase they choose otherwise.

Guests are requested not to bring any eatables or outside food, soft drinks, liquors, cigarette and other tobacco products and drugs.

The right to admission is reserved with the management.

Cancellation policy is applicable.
